New patient registration
We are honored to have you as part of our team. In order to provide you and your family the best possible care, it is very important that we have access to your child’s previous medical records before scheduling the first appointment. This will allow us an opportunity to review the records and be familiar with your child’s past medical history, medications, and immunizations before meeting.
We understand obtaining records can be challenging, but rest assured we are here to help. After completing the registration form, you will be contacted within two business days with additional information about how to transfer your child’s medical records securely and schedule a first appointment with one of our medical providers. Please also visit our Forms and Policies section online and complete the New Patient Registration form found there; this can also be faxed to our office at 781-435-0792.
If you have questions about this process, or have issues with computer or fax access, please call the office at 781-451-0072 and speak with our front desk staff. We want to make this process as simple as possible. We look forward to meeting you at our office in Stoneham, MA!

Information for parents
Whether related to school, sports, friends, or family your goals are always the primary focus at our office. We are board certified pediatricians and nurse practitioners proud to provide a medical home where families are known, supported and respected. We have treated children from birth to 22 years of age over our combined more than 30 years of experience.
We work as a team to help patients manage their healthcare needs. We love what we do and know that this shows in each interaction we have with our patients and their families. We are very grateful for how well our practice has been received in Stoneham and the surrounding areas. Insurances we accept
Commonwealth Pediatrics currently accepts many major insurers. Please contact your insurance provider first to ensure our office is in network, and match your PCP with the one you see in our office.
We currently accept the following plans:
• Aetna
• Blue Cross Blue Shield
• Cigna (excepting Local Plus)
• Coventry
• Fallon (commercial plans only)
• Harvard Pilgrim
• Humana
• Multiplan
• Three Rivers
• Tricare
• Tufts Health Plan (excepting all Steward plans)
• Wellsense ACO through BCH for MassHealth
• Unicare
• United Healthcare
We also accept MassHealth, though all of our MassHealth patients must enroll in the following plan: Wellsense Boston Children’s ACO. Effective 1/1/25, we will no longer accept Tufts Health Direct Plans offered on the Massachusetts Health Connector.
